  2. Copy of Philippine Birth Certificate
  3. Report of Marriage OR Marriage Contract (if married in the Philippines) issued by the National Statistics Office (NSO) authenticated by the DFA-Manila
  4. Lebanese passport
  5. Philippine Passport
  6. Residence permit/Iqama of the applicant. (If applicant have a minor child, submit Report of Birth, Lebanese Identity Card, Philippine and Lebanese Passports of the beneficiary).
