REPORT OF BIRTH

1. Duly accomplished Report of Birth form

2. Birth Certificate of the child (Translated in English and authenticated by the MFA ELebanon)

3. PSA Report of Marriage or PSA Marriage Certificate of parents

4. Passports of parents.

Additional Requirements for Late Registration (1 year and above):

1. PSA Certification of Negative Birth Record

2. Affidavit of Delayed Registration

3. Affidavit of Two Disinterested Persons and Copies of their Valid Passports/IDs

For Unmarried Parents:

1. If the child is using the surname of the father, submit the following:

a. Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother

b. Affidavit of Acknowledgement of Paternity to be executed by the father.

2. If the child is using the surname of the mother, submit the PSA Birth Certificate of the mother.

Bring the original and three (3) photocopies of the documents.

REPORT OF MARRIAGE

1. Duly accomplished Report of Marriage form

2. Marriage Contract/Certificate (Translated in English and authenticated by the MFAE in Lebanon);

3. PSA Birth Certificate of the Filipino National

4. Passports of both parties

Additional Requirements for the Late Registration (1 year and above)

1. PSA Negative Certification of Marriage Record

2. Affidavit of Delayed Registration

3. Affidavit of Two (2) Disinterested Persons and Copies of their Valid Passports/IDs

Bring the original and three (3) photocopies of the documents.

SOLEMNIZATION OF MARRIAGE

(Done at the Embassy for both Filipino Nationals)

1. PSA Birth Certificates of both parties

2. PSA Certificates of No Marriages (CeNoMar) of both parties

3. Passports of both parties

4. Four (4) passport-size photos of both parties

5. Affidavit of No Legal Impediment to Contract Marriage

6. Application for a Certificate of Legal Capacity to Contract Marriage

7. Notice of Intended Marriage (to be posted at the Embassy Premises)

8. Marriage License
