GENERAL INFORMATION  (Please Read):

The Philippine Passport is at all times the property of the Republic of the Philippines. It must be surrendered upon demand by an authorized representative of the Philippine Government. 

Alteration, addition, or destruction of contents is strictly prohibited. Any unauthorized change will render a passport invalid and will have accompanying penalties. 

Using the passport as collateral is against the law and will automatically render the passport cancelled. 

The information provided in the passport application form must be true and correct. The documents that will be submitted should be complete and authentic. Under the law, making false statements in the passport application form, furnishing falsified or forged documents in support thereof are punishable by law.

Requirements for Passport Application:

I. RENEWAL OF PASSPORT (ADULT APPLICANT)

A. GREEN PASSPORT; any older passports; and MACHINE READABLE PASSPORT (Maroon)  are TREATED AS A NEW APPLICATION

 CORE REQUIREMENTS

  • Personal Appearance
  • Accomplished application form
  • Bring the original passport
  • Original copy of Philippine Statistics Authority (PSA, formerly NSO) Authenticated Birth Certificate on Security Paper:

Married Females must also submit Original copy of PSA Authenticated Marriage Contract on Security Paper or Report of Marriage

  • Any of the following Valid IDs with one (1) photocopy:
    1. Social Security System (SSS)/Government Service Insurance System (GSIS), Unified Multi-Purpose Identification (UMID) Card
    2. Land Transportation Office (LTO) Driver’s License
    3. Professional Regulatory Commission (PRC) ID
    4. Overseas Workers Welfare Administration (OWWA)/ Integrated Department of Labor and Employment (iDOLE)
    5. Commission on Elections (COMELEC) Voter’s ID or Voter’s Registration Record from COMELEC Head or Regional Office
    6. Philippine National Police (PNP) Firearms License
    7. Senior Citizen ID
    8. Persons with Disabilities (PWD) ID
    9. School ID (for students)
  • Processing fee of US$ 60.00

B.  E-PASSPORT

CORE REQUIREMENTS

  • Personal Appearance
  • Accomplished application form
  • Current ePassport with photocopy of data page
  • Original PSA Authenticated documents that will support the change of name (if applicable)

-          Marriage Contract

-          Annotated Birth Certificate

-          Annotated Marriage Contract to show annulment/divorce/court ordered instruction

-          Death Certificate of Spouse

  •  Processing fee of US$ 60.00

In addition to core requirements, the following SUPPORTING DOCUMENTS will be further required for the following cases:

  1. In case of a LOST VALID ePassport (TREATED AS NEW APPLICATION)
  • Photocopy of the lost passport
  • PSA Authenticated Birth Certificate on Security Paper and a photocopy
  • Married Females: Original copy PSA Authenticated Marriage Contract on Security Paper
  • Any of the following Valid IDs with one (1) photocopy:

-    Social Security System (SSS)/Government Service Insurance System (GSIS), Unified Multi-Purpose Identification (UMID) Card

-    Land Transportation Office (LTO) Driver’s License

b. In case of a LOST EXPIRED ePassport (TREATED AS NEW APPLICATION)

  • PSA Authenticated Birth Certificate on Security Paper and a photocopy
  • Married Females: Original copy PSA Authenticated Marriage Contract on Security Paper
  • Any of the following Valid IDs with one (1) photocopy:

-    Social Security System (SSS)/Government Service Insurance System (GSIS), Unified Multi-Purpose Identification (UMID) Card

II. MINOR APPLICANT

  • NEW

-          Accomplished Application Form

-          Personal Appearance of minor applicant and either parent or authorized adult companion

-          PSA Birth Certificate or Report of Birth with complete attachments

-          Passport or Valid Government issued ID of either parent

-          School ID (if available)

  • RENEWAL

-          Accomplished Application Form

-          Personal Appearance of minor applicant and either parent or authorized adult companion

-          PSA Birth Certificate

NOTE: RENEWAL OF PASSPORT WILL REQUIRE MINIMUM OF 30 DAYS AND MAXIMUM OF 60 DAYS
